If you are operating at a hospital we do not yet have credentialing at, we ask that you reach out through the submission form below and send the following:

​

• A "Letter of Need" that we can submit to the hospital to expedite the credentialing process. We have a template we can provide you with if needed. 

• The facility representative's contact name and email address.

• Surgical procedure to be completed.

• Dates and times needed if this is known at the time of submission.
